Kahu Malama Nurses is seeking Lead Caregiver Assistants to join our growing and dynamic team!

Job Description:

  • Assures that the rights of participants are protected, including protection from abuse and neglect, dangerous or hazardous conditions and by following established reporting procedures.
  • Assures confidentiality of all participants protected health information and records from unauthorized access or dissemination.
  • Develops and implements individual treatment plans for each participants based upon the IPP or ISP.
  • Prepares class schedule and lesson plans in a timely manner.
  • Orders and dispenses medications to participants, as needed.
  • Supports participants in performing activities of daily living skills such as meal preparation, eating, grooming, toileting, dressing, and utilization of assistive devices (e.g. wheelchairs, walkers, etc.) in accordance with the individuals IPP or ISP.
  • Assists in maintaining properties, grounds, and surrounding area in a neat, clean,

and sanitary condition by performing cleaning duties, such as sweeping, mopping, washing, and scrubbing on a regular basis.

  • Prepares program reports as requested by the Center Manager.
  • Interacts and attends meetings with parents, guardians, interdisciplinary team members, and other related persons and programs to benefit the individuals.

Position Requirements:

  • Must have a valid Drivers License
  • Must have a smartphone
  • Must be at least 18 years old
  • Working knowledge and application of normalization principle and ICF-MR-C program experience
  • Ability to safely drive a van
  • Strong verbal, written, and interpersonal skills
  • Ability to operate assistive devices including Hoyer lift, wheelchair, walker, and gait belt
  • Ability to intermittently lift, push and pull up to 50 lbs., with minimal assistance
  • Current work clearances which may include, but are not limited to, PE, initial 2-step PPD, annual PPD, CPR, First Aid, criminal history, APS & CAN registries, FBI fingerprinting and a drivers abstract with an acceptable level of infractions. Hepatitis B vaccination optional but recommended
