Job Description


Working out of the Office of Ambulatory Care and Population Health, the Access Implementation Specialist will play a critical role in improving access for patients to primary care and specialty care appointments across all ambulatory care sites. Specifically, this role involves working to address upstream barriers to patient care access including but not limited to template building, scheduling, referral pathways, and clinical practice standards. The Access Implementation Specialist will also provide ongoing change management and quality improvement support for each site that they work with as related to workflow redesign, development of new standard work and data analysis to assess progress and sustained improvement in patient access.

General tasks and responsibilities will include:

  • Provide in-depth support and education to sites and template managers to improve template builds, scheduling, and practice management with ongoing quality assurance and checkpoints on progress.
  • Use data to understand areas for improvement in access at assigned sites and departments and implement different strategies to improve access based on these data.
  • Work closely with Referrals team and site-based providers and support staff to understand barriers to specialty care access and assist in developing specific strategies to address these special needs.
  • Work closely with operational and clinical leads both within Central Office and at the facilities to ensure the successful implementation of any changes to templates, schedules or workflows related to access as above.
  • Serve as a liaison and colleague between sites and Contact Center staff to alleviate barriers to scheduling and access.
  • Coordinate with other teams on related initiatives in quality improvement and practice transformation as needed to ensure clear, consistent, and achievable goals are set and met.
  • Work closely with Epic Ambulatory and Epic Cadence teams to communicate required EMR changes to facilitate access for patients.
  • Conduct on-site meetings throughout the City with staff and leaders at hospital-based ambulatory care clinics throughout the City in order to advance the goal of improving patient access.

Minimum Qualifications


1. A Masters Degree or advanced clinical degree from an accredited college or university and two years of experience serving as a lead in soliciting, defining, and managing requirements for internally and externally developed software in a clinical, allied health, non-allied health, behavioral health, information technology, information services, business, analytics or related area; or, one year of experience in a relevant clinical and/or healthcare administration role/function; or
2. A Baccalaureate Degree from an accredited college or university and five years of experience as described above; or
3. A satisfactory combination of education, training, and experience.
4. Specialty certification(s) issued by a national commercially available program, state, professional society, academic or technical institution in an area(s) listed above, may be credited on a month-to-month basis toward the required work experience for a total of one year.
