The Bilingual Telephonic Care Coach 1 / Specialist assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members.

Responsibilities

Bilingual Telephonic Care Coach 1

  • Employs a variety of strategies, approaches and techniques to manage a member's health issues.
  • Identifies through a screening process for potential eligibility for state and federal benefits.
  • Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning
  • Working with members and potential members over the phone

Required Qualifications

  • Bilingual English/Spanish with ability to pass a language assessment
  • 1 or more years of experience in telephonic intensive environment (inbound & outbound)
  • 1 or more years of customer service experience
  • Current valid Driver’s license required & ability to travel up to 50% at times in surrounding counties
  • Team player, ability to work with tight schedules, multiple priorities as well as work independently
  • Strong proficiency in Microsoft Outlook, Excel, Word & PowerPoint

Preferred Qualifications

  • Prior experience with Medicare & Medicaid recipients
  • Previous experience with electronic case notes and documenting in multiple computer applications/systems
  • Experience with health promotion, coaching and wellness
  • Microsoft Teams

Hours

  • Monday-Friday 8AM-5PM (flexible based on Community Center needs)
  • Must be able to work Mon-Fri between the hours of 8AM-7PM but be flexible with your hours based on business needs and hours could include weekends/holidays during the Medicare Annual Election Period, September 16th- December 8th.

Bilingual Assessment

  • Any Humana associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.

Job Type: Full-time

Pay: $48,400.00 per year
