The Bedford-Stuyvesant Family Health Center (BSFHC) is a patient-centered, team based multi-specialty practice in central Brooklyn. BSFHC has been providing affordable, comprehensive, outpatient medical and dental services in an outpatient setting for over 40 years. As a Federally Qualified Health Center (FQHC), BSFHC serves all of the primary health care needs of families in the heart of North and Central Brooklyn. We are seeking an Outreach Health Educator. This position joins the HIV Prevention Team in the Wellness Center to support activities to promote and improve the health for people at risk of HIV/AIDS in addition for people with HIV in the wider community. The Outreach Prevention Educator aims to increase health-related issues in our community to improve health making decisions. The program will achieve this goal by facilitating health fora, conduct presentations, and link community members to insurance and health-related services in addition to increasing referrals to HIV prevention and medical services.

As an Outreach Prevention Educator, you will be responsible for understanding how the role interplays with the other functions in the Wellness Center and optimize the delivery of healthcare to all clients and patients of the Health Center. This position must be educated in the various forms of HIV and Hepatitis C prevention, treatment and models of care coordination.

Required fieldwork about 75% of the time and take mass transit to travel throughout New York City.

Bilingual Spanish required.

Grant-funded: Contingent upon continuation of grant funding from year to year.

RESPONSIBILITIES:
Outreach Prevention Educator:
  • Develop partnerships at the community-level to identify individuals who are underserved, uninsured, out-of-care, especially immigrants and other hard-to-reach populations, as well as those in need of access to HIV care, behavioral health, and chronic disease management.
  • Conduct outreach activities at various community organizations and health information settings to raise awareness about health and insurance coverage and to provide information about the services at the health center and through the supporting programs.
  • Conduct outreach presentations at various events and at outreach settings throughout Brooklyn.
  • Conduct rapid HIV testing to all persons (internally and in community settings) who are interested in PrEP and perform risk assessments for everyone who expresses a need to start taking PrEP as a medication to prevent HIV.
  • Utilize social marketing and social media tools and partners to identify and contact out to hard-to-reach population who are underserved and uninsured.
  • Support the PrEP, HIV & HEP C programs by referring partners and community to accessing prevention services particularly among MSM of color, Black women and sexual and gender minorities (SGM).
  • Conduct outreach events, meetings that are in keeping with core job requirements.
  • Submit clear and accurate reports based on data collection, outreach activities, and patient engagement across all service-levels.
  • Maintain contact with other community-based organizations, both HIV-specific and non-HIV-specific, to deliver Develop and maintain an outreach database is a necessary requirement for this position.
  • Keep supervisor informed of all problems, challenges and conflicts related to job; and
  • This position requires the candidate to be on the field about 75% of the time and take mass transit to travel throughout Brooklyn.
KNOWLEDGE, SKILLS & ABILITIES:
Successful applicants MUST be able to show how their previous experience in connecting and reaching underserved and hard-to-reach populations. Must have demonstrated ability to work independently and document all work performed. The successful candidate must have an understanding of the community's cultures and ethnicities and knowledge of community assets and resources necessary to take advantage of its opportunities and to respond positively to its challenges. Knowledge of Brooklyn-based systems of care. Ability to deal effectively with a variety of people and work in an affirming team environment. Sensitivity to the needs and situations of a multi-cultural population from a variety of income levels. Ability to prioritize goals and objectives to deliver mission-critical services, on time.

  • Provide services to any designated service delivery site.
EDUCATION, EXPERIENCE, CERTIFICATIONS:
  • Bachelor's degree required.
  • Experience working with HIV/AIDS, LGBTQ, and communities of color.
  • Bilingual (Spanish/English)
  • Efficient in rapid testing technologies and counseling
  • Ability to provide community health information to the most vulnerable populations in Brooklyn.
Contract details: Full-time contract for 1 year.

Salary Range: $ 45,000 - $52,000
