This culturally and geographically connected individual serves as a link between the underserved and rural communities and existing community resources, provides outreach, engagement, provides assessments and develops care plans for members. Through home visits whenever possible, the LCM assists members in overcoming barriers to health, social services, education and employment by connecting them to appropriate community resources. The LCM empowers members to navigate medical, behavioral health and social services by removing barriers to these services.
Qualifications:
- Bachelor's degree in a related field or will have at least three years’ experience working directly with unhoused individuals, the frail elderly, those with chronic health conditions and/or SMI/SMU.
- Care management experience with the frail elderly, those with chronic health conditions and/or SMI/SMU a plus
- Must be able to travel to in-home visits with our members in San Luis Obispo County. (Although the job is "remote", you must be physically present in San Luis Obispo to make home visits to our members).
- Bilingual English/Spanish preferred
- Must have reliable transportation, insurance (100,000/300,000) limit, clean DMV record. Will use own vehicle to provide in-person services. (Mileage is reimbursed at IRS rate, which is 65.5 cents per mile).
Job Duties:
- Conducting comprehensive assessments of each member's medical and social needs (SDoH).
- Develop a care plan in collaboration with the member and caregiver when appropriate, assess the caregiver as appropriate, for caregiver role-strain and address this in care plan.
- Provide closed loop referrals to appropriate community resources that will support the member to continue living in the community, coordinate care across multiple healthcare settings and providers to reduce duplication of services and ensure that appropriate services are being provided.
- Provide education and support to members and their families as needed and refer to consultant RN, and other ADHC professionals to address any health related needs and education
- Monitor member progress and adjust care plans as needed.
- Document all care management activities in the member's medical record.
- Provide outreach to members who have been referred to our program and support them to enroll in the ECM program. This is accomplished through in-person, phone, mail, and telehealth.
- Job is primarily in the San Luis Obisipo area, with one day a week or less meeting with team in Santa Maria.
- Training will be in person, in the Santa Maria area.
Job Type: Full-time
Pay: $24.00 - $29.00 per hour
