Job Summary and Qualifications

The Care Assure Navigator will be responsible for duel navigation for both the lung and traditional Care Assure programs. The Navigator manages the daily operations and workflow of both lung and traditional programs in conjunction with physician(s) and administrative leadership. The focus of the lung role will be to manage and follow up on lung nodules found through various avenues including but not limited to high-risk lung screenings, incidental findings in the Emergency Room, Outpatient, and Inpatient settings. The lung navigator will coordinate the next level of care for patients with lung nodules and assist patients, clinicians and program stakeholders in the collection, reporting and presentation of program outcomes. Traditional navigation is responsible for the programs and resources that support the navigation of patients through complex healthcare environments for the optimization of care and outcomes. The navigator facilitates patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process. The duel navigator role serves as a point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services.
