Care Coordinator Job at Right Hand Senior Care
Care Coordinators are key employees of the company and are selected for their compassion, dedication, work ethic, skill level, time management and communication abilities. The purpose of this role is to ensure that RHSC direct service workers are trained and qualified to perform the skills needed to follow the plan of care for RHSC clients. He / She is also responsible for ensuring that employees are properly oriented on RHSC policies and procedures. He/she will work closely with the RHSC team to ensure client’s are staffed properly and satisfied with the services provided.
Key Duties:
Train all new employees onsite on their first shift with the company.
Train and/or re-train employees on site with new and existing RHSC clients.
Ensure that caregivers are properly trained to care for the client. This includes documenting onsite training sessions, directly observing caregivers performing tasks and referring additional training needs to management.
Routine pop-ins on staff to ensure services are being provided.
Verifying DSWs are correctly and timely reporting critical incidents within the home.
Supporting the team with staffing issues and serving as a back up worker in the event a shift cannot be covered or in the event of an emergency.
Reviewing progress notes to confirm the plan of care is being followed.
Making sure caregivers complete a bi-weekly or, in some cases, weekly inventory of client supplies, food and medication for specific high-level clients.
Verify DSWs are assisting with scheduling doctor appointments and ensuring transportation is organized to and from the appointments.
Communicating with a remote administrative team.
Attending and participating in planning meetings, training sessions and management meetings for the clients.
Assisting operations manager with caregiver’s infractions and disciplinary procedures.
Strategizing with the Recruiting Team to recruit staff where needed.
Being a good example and leader for the care team in the client’s home by abiding by RHSC’s policies and procedures at all times, wearing the RHSC uniform, being on time for shifts and training sessions, and most importantly, keeping a positive attitude.
Participating in On Call rotation with the RHSC team per the schedule. Stepping in if another Care coordinator is ill or on approved time off during his/her rotation.
Requirements:
Reliable Transportation
Clear background check
CNA certification, or college coursework (may be waived with extensive experience)
Full time position/ 40+ hours per week
At least 2 years caregiving experience
Compensation:
Salary range is $35,000-$37,000 yearly
