This Care Management Associate (CMA) position is fully remote and employees can work from any state, however, preference is for candidates in the time zones of PST, MST, or CST.
Normal Working Hours: Monday through Friday between the hours of 11:30am-8:00pm EST. There are no weekends or holidays currently. The hours of operation may change in the future per the needs of the business.
The CMA supports the tasking needs of the Transform Diabetes Care team (TDC) in Aetna’s Commercial Care Management division.
The Care Management Associate is an integral part of the Transform Diabetes Care team. The CMA provides comprehensive coordination of medical services including program enrollment to promote the improvement of health and wellness outcomes for diabetic members. The Care Management Associates support Certified Diabetes Care Educators by performing non-clinical, administrative duties.
As part of the patient management team, the CMA daily functions include managing in-bound call activity and coordinating care with appropriate nurses or registered dietitians, verifying eligibility, transferring callers to appropriate clinical staff, and providing excellent customer service.
The CMA will use software programs to enter data, research information and document activities.
The CMA will make outreach calls to provide health screening reminders for eligible members, order diabetic monitoring equipment, assign cases, process referrals, and provide appointment reminders for established members.
The job responsibilities require the ability to learn multiple processes and complete them consistently, efficiently and in compliance with detailed procedures.
Pay Range
The typical pay range for this role is:
Minimum: 18.50
Maximum: 34.60
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

Required Qualifications
- 1 year of customer service experience

- Experience as a Medical Assistant or Office Assistant
- Effective communication skills using a telephone
- Organization skills
- Familiarity with basic medical terminology and Managed Care concepts
