Job title: Case Manager - Queens, NY
Description
The successful candidate will provide a full range of on-site, person-centered, strengths-based services to a caseload of approximately 20-25 clients with the objective to help people live fulfilling lives in the community.
Responsibilities
- Conduct initial intake assessments and act upon critical needs appropriately
- Utilizes assessment tools to identify support service needs and make immediate referrals
- Orient clients to the shelter program, requirements and consequences
- Provide client with Independent Living Plan working in collaboration with other
- Social Service providers and ensure the delivery of comprehensive support services.
- Assist non-working clients to obtain Public Assistance
- Complete psycho-social assessments within the first 30 days of intake
- Track and monitor client progress
- Meet with clients to address issues, challenges and successes to assist in strengthening their
- capacity for long-term self-sufficiency
- Maintain client files to include conversations, warnings and progress reports and documenting all incidents
Requirements
- Bilingual Spanish, French, Russian, and Arabic candidates are preferred
- Strong interpersonal skills with the ability to be compassionate and firm and maintaining confidentiality at all times
- Knowledge of community resources
- Ability to solve problems, make decisions, resolve conflicts and listen
- Ability to deal calmly in crisis situations.
- Proven ability to work collaboratively well with diverse groups
- Proven ability to handle multiple tasks effectively under pressure
- Strong organizational skills, detail-oriented, and efficient
- Maturity, integrity, and sound judgment
Education
- High School Diploma/General Equivalent Diploma with 8 plus years of direct social service experience
- Associate degree in Human Service, or related field with 4+ years of direct social service experience
- Baccalaureate degree in Social Work, Psychology, Sociology, or related field with 2+ years of direct social service experience
Location: Queens Boulevard, New York.
Schedule:
- 9am - 5pm: Tue - Sat
- 8am - 4pm on Sun & 9am - 5pm: Mon - Thu)
- 12pm - 8pm: Mon - Fri
- 2pm - 10pm: Mon - Fri
Financials: Payrate: $21.63/ph
