NKADD, Division of Aging and Disability Services, is seeking a full-time Case Manager to provide case management for individuals with physical, intellectual, and/or developmental disabilities in our community. A Bachelor’s Degree in a human services field is preferred.

Job Summary:
- NKADD seeks a qualified individual as a full-time Case Manager for the Participant Directed Services Medicaid Waiver program.
- This position works with clients having physical, intellectual, and/or developmental disabilities in our community. Responsibilities include the development and monitoring of individuals' plan of care, working to help clients maintain independence in their home and community.
- Act as an advocate to clients to ensure they receive necessary services to maintain their independence in their home and in the community.
- Develop knowledge and establish good working relationships with community support resources and services within the community.
Requirements:
- Bachelor’s degree in Social Work or related Human Service Field, or RN preferred. 1-year minimum experience in the field of health and human services is preferred.
- Customer Service orientation is necessary.
- Excellent organizational, verbal, written, and interpersonal skills are required.
- Applicants must possess a valid driver’s license and have the ability to successfully pass a background check.

Pay: From
$37,000 to $39,000 per year.
