Join our talented and dedicated Case Management team and make a positive impact on the lives of others! We have 2 openings for a full-time Case Manager to coordinate client services in the greater Manchester area. This includes evaluating client needs, prioritizing the level of service coordination, determining appropriate and cost-effective services, tracking expenses and documenting outcomes.

This position is currently remote. May become hybrid in future.

You will:

· Demonstrate knowledge of NH Regs (He-M 503, 507, 521, 522, 525, 1001, 1201) and ensure compliance with regulations

· Develop a Service Agreement including goals, objectives, methodology and timelines based on assessments

· Continuously review services, assessing appropriateness and progress toward goals and/or treatment

· Coordinate RFP process to create a new or different service provider as necessary

· Assist clients in communicating their needs, desires and preferences, as necessary

· Negotiate service contracts with vendors that demonstrate quality services

· Demonstrate an understanding of client budgets

· Research community organizations to reduce costs and increase client supports

Qualifications:

· Bachelor’s Degree preferred; consideration may be given to candidates with an Associate’s Degree and a minimum of 2 years’ experience working in human services

· 1 – 2 years’ experience working in human services preferred

· Proficiency in various software applications including Word, Excel, and Access

*Guidelines:

· Sign-on Bonus is effective for Case Managers until Dec. 1, 2022

· The bonus is effective for all eligible employment applications and resumes

· Bonus pay-out will occur after employee completes a favorable 30-day probationary period

Job Type: Full-time

Pay: $38,000.00 - $38,500.00 per year
