Case Manager (Truancy)- ($18hr - $21hr).
Job Description:
Summary: Under the Senior Vice President's and President's authority and the Program Supervisor's supervision, the Case Manager is responsible for contact with and treatment services to youth and families; contact with line community agency personnel, and administrative documentation of services provided Click Here to Apply!
Essential Functions:
- Successfully engage and maintain youth and families through the treatment process.
- Provide effective case management services to clients in offices, client homes, schools, and other settings as appropriate within the context of outreach services and according to treatment plans.
- Identify, link, and coordinate all necessary services.
- Implement services in cooperation with the youth's therapist, meeting at least one time per week to coordinate services.
- Review services every month and document the effectiveness of services in the plan and clients' continuing need for services.
- Provide regular ongoing case coordination and collateral contact with other community agencies also involved in individual cases.
- Achieve stated treatment goals with assigned families.
- Represent the program in treatment conferences with other agencies professionally.
- Provide a quality representation of services with an individual family in court when subpoenaed.
- Meet at least one time per week with his/her assigned supervisor individually or as part of a team.
- Provide crisis response to families in need including providing program on-call responsibilities which include responding by phone within 15 minutes of the contact and going to the family home or community site if the situation cannot be resolved by phone.
Key Functions:
- The case Manager's time will be spent providing direct, in-person services according to established program time frames and protocols.
- Complete the Home Based Safety Assessment and Plan according to program time frames and protocol.
- Complete and submit to the case file all clinical documentation, progress reports, and case recordings in a professional manner within established program time frames.
- Complete and maintain service authorizations.
Other Duties
- The accurate and timely completion of personnel material (e.g. mileage reimbursement, timesheets, etc).
- Assess and collect client fees.
- Complete other related tasks as appropriate or assigned.
- Support agency values and goals.
- Timely completion of training by program standards.
Preferred Qualifications
- A mature person with sound judgment and represent an appropriate role model to youth and families.
- He or she must be an individual who has never been convicted of any child abuse offenses.
- An Associate's Degree with experience is necessary. A Bachelor's Degree is preferred.
- Training and experience in crisis intervention and one year of experience in a CASSP system are mandatory for Case Managers in the Children's Behavioral Health Component.
Physical Requirements:
