Job description:
The Case Manager is responsible in providing recovery-oriented rehabilitation services in group or individual settings. Case Manager duties include assessing, planning, implementing. monitoring and evaluating actions required to meet the client's health and human services needed.
Responsibilities:
- Create ISP's, Discharge Plans, Annual Assessment for all new residents
- Schedule and participate in monthly ART meetings with clinical and outpatient teams
- Complete monthly CCR to identify resident care need and to justify stay
- Implement and review treatment plans as needed
- Collaborate with additional treatment providers and placement agencies
- Ensure all documentation is completed in a timely manner
- Coordinate and accompany residents to provider appointments
Qualifications:
- A High School Diploma/GED and three years of relevant experience in a behavioral health setting or an
- An Associates and or bachelor’s degree and two years of relevant experience in a behavioral health setting
- Must be at least 21 years of age.
- Minimum typing speed of 25 wpm.
- Some assignments require an Arizona Fingerprint Clearance Card
- Current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR)
Job Type: Full-time
Pay: $20.00 - $24.09 per hour
