Job Title: Case Manager Registered Nurse

Job Summary:

We are seeking a highly motivated and compassionate Case Manager Registered Nurse to provide patient-centered care coordination and management to individuals across the healthcare continuum. The Case Manager RN will be responsible for assessing patient needs, coordinating care, and collaborating with healthcare providers to ensure high-quality, efficient care delivery. The successful candidate will have strong critical thinking skills, excellent communication skills, and a commitment to improving patient outcomes.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' medical, social, and behavioral health needs.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Coordinate patient care across the healthcare continuum, including transitions of care, referrals, and follow-up appointments.
  • Monitor patient progress, identify barriers to care, and adjust care plans as needed.
  • Advocate for patients and their families, ensuring that they receive appropriate care and services.
  • Collaborate with healthcare providers and community resources to ensure patients receive high-quality, efficient care.
  • Educate patients and families on disease management, self-care strategies, and available resources.
  • Document patient interactions, care plans, and outcomes in electronic health records.
  • Participate in quality improvement initiatives, including data analysis and performance improvement projects.
  • Stay current with new and emerging healthcare technologies, treatments, and trends.

Requirements:

  • Current licensure as a Registered Nurse in the state of practice.
  • Minimum of 2 years of experience in case management or care coordination.
  • Strong understanding of healthcare delivery and care coordination principles and practices.
  • Ability to assess and manage complex medical, social, and behavioral health needs.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong attention to detail and ability to manage multiple tasks simultaneously.
  • Ability to maintain composure and professionalism under pressure.
  • Familiarity with electronic health records and other healthcare information systems.

Preferred Qualifications:

  • Bachelor's degree in Nursing or related field.
  • Certification in Case Management or related field.
  • Experience in a hospital or clinical setting.
  • Familiarity with insurance and reimbursement policies and procedures.

Physical Demands:

Working Conditions:
