Summary:
Provides assessment, intervention, discharge planning, resource utilization and managemnt, program planning, data collection and analysis in order to optimally meet the biopsychosocial needs of all consumers. Facilitates appropriate patient movement through levels of care throughout the continuum.
Responsibilities:
1. Assessment Criteria A: Completes initial assessment to determine patients discharge needs and case management intervention. Interviews client/family to assess needs by completing assessment form. Assessment Criteria B: Incorporates current treatment plan to determine realistic goals, objectives and alternatives as indicated by documentation.
2. Planning Criteria A: Includes the client/family as decision maker and goal setter. Initiates and implements plan modifications as indicated by monitoring and re-evaluation thru client/family meetings.
3. Utilization Criteria A: Utilizes CDMP process to establish DRG. Utilization Criteria B: Utilizes Medicare guidelines to determine intensity of illness and intensity of service. Utilization Criteria C: Monitors criteria for admission and continued stay. Ensures appropriate utilization of resources. Utilization Criteria D: Establishes Conncur number on Medicaid admissions. Establishes client benefit for post acute services and equipment as indicated.
4. Discharge Planning Criteria A: Demonstrates knowledge of community services and programs. Discharge Planning Criteria B: Establishes discharge plan in collaboration with client, physician and care team. Discharge Planning Criteria C: Provides education to client/family related to the discharge plan. Provides opportunities for additional education by acting as liaison between client/family and care team.
5. Professional Growth Criteria A: Pursues professional growth by attending staff meetings and inservices. Professional Growth Criteria B: Attends a minimum of one continuing education seminar per year as documented by course certification or obtains continuing education hours by completing appropriate job related materials.
6. Leadership Criteria A: Contributes to meeting the fluctuating needs of the organization as evidenced by flexibility in assignment. Leadership Criteria B: Covers other similar services within the same level of care.
7. Quality Improvement Criteria A: Participates in continuous QI activities. Quality Improvement Criteria B: Meets all QA criteria as outlined in Case Management QA Plan.
8. Performs other duties as assigned.
9. Fulfills all compliance responsibilities related to the position.

Required: BSN or equilvalent educaton and experience. Connecticut MSW required. Broad based clinical background and hospital experience required. Case Management Certification preferred. Certification required upon meeting eligibility. Must be able to deal with a high degree of public and patient contact and interaction. Must be able to make independent decisions, perform concurrent tasks, manage stressful situtations and balance multiple priorities. Meets all requirements for clinical ladder.
