POSITION TITLE: CNA (2P-10P)

POSITION SUMMARY: The purpose of this position is to provide direct care to residents under the supervision of a licensed nurse in accordance with facility policies and procedures. Reports resident needs and concerns to a licensed nurse.

RESPONSIBILITIES:

· Assists the RN, LVN and other members of the nursing team in the care of the patient.

· Provides good quality resident care by performing routine nursing care and services to defined patient groups and their families.

· Properly plans and organizes work for the assigned shift to ensure patient receives appropriate quality care (passing ice water to ensure hydration, provide showers as assigned and ensure shower room cleanliness, rounding often, meal service attendance, room cleanliness (beds made), attend to resident grooming needs, etc.).

· Provides emotional and social support to residents.

· Demonstrates clear and effective communication skills. Asks questions and seeks guidance when not sure of what to do.

· Maintains a positive and professional demeanor toward residents, visitors, families, and co-workers that contributes to a positive work atmosphere.

· Accurately completes and documents assignments in the Electronic Medical Records system (PCC).

· Perform other duties as assigned/required.

QUALIFICATIONS:

• High School Diploma or General Education Degree; or one to three months related experience and/or training; or equivalent combination of education and experience.
• Active CNA license required.
• A valid TB test and BLS/CPR card.
• Ability to lift at least 51 pounds on a regular basis and physically able to help residents who require physical assistance.
• Read, write, speak and understand the English language.
• Ability to pass a criminal background check and drug screen.

OTHER QUALIFICATIONS:

· Must possess a general appreciation of seniors and an understanding of the aging process, as well as be aware of its challenges.
