Position: Interdisciplinary Group Clinical Manager
Position Summary:
Responsible for the overall management of the Interdisciplinary Group. Provides overall supervision of staff as well as providing support and assistance as needed. Facilitates patient care activities.
Qualifications:
A Bachelor of Science in Nursing is preferred. Must be a graduate of an accredited school of nursing and be currently licensed as a Registered Nurse in the State of Florida or Alabama, depending upon assignment. Hospice experience is preferred and/or five (5) years of recent experience in a medical/surgical setting, a hospital, oncology or geriatric facility. One year of clinical supervisory experience preferred. This position requires an individual with excellent communication skills, displayed management and leadership skills and an ability to interact successfully with members of the healthcare community and other community leaders. Reliable transportation, valid state of Florida or Alabama driver’s license, as applicable depending on assignment, and current automobile liability insurance is required. Must possess current CPR certification.
Job Relations:
A. Reports to: Regional Director of Operations
B. Direct Staff Supervised: IDG members including RNs, LPNs, Social Workers, Hospice Aides, patient/family units, and others as assigned.
Main Duties and Responsibilities
A. Manage the Interdisciplinary Group, ensuring appropriate delivery of Hospice services to patient/family units and care partners. Coordinate and supervise field staff to provide optimum quality patient/family care. Direct Administrative Staff in the management of office business.
B. Ensure appropriate development and maintenance of a written plan of care for each patient/family unit and updates twice monthly or more often if needed. Make periodic visits to patient/family units and care partners for the purpose of:
1. Evaluating nursing care, volunteers, the interdisciplinary group and general quality of service provided.
2. Performing bi-annual supervisory visits with all field nurses on team.
3. Assessing to ensure that appropriate and adequate psychological, emotional, and spiritual support is provided to the patient/family unit.
C. Implement staffing patterns taking into consideration budget factors, patient acuity and FTE reports. Monitor productivity and manage to expectations.
D. Conduct regularly schedule IDG meetings, in conjunction with the IDG Teams/Branch Office Manager.
E. Provide supervision, support, and encouragement to IDG members.
F. Manage Interdisciplinary Group budget.
Manage the financial operations of the team to enhance cost-effective consciousness to include face-to-face visit compliance and hospice item set completion. Responsible for timely payroll and other financial information to the business office (i.e. change of status forms, time sheets, etc). Responsible for timely submission of patient account information (change of status, election statements, facility status forms, etc.)
G. Evaluate performance of the IDG and other assigned administrative staff. With disciplines other than nursing, the appropriate Corporate Program Director will be involved with the evaluation process.
1. Oversee preceptor field orientation for new employees. Conduct ride-along on all RN/LPNs at completion of orientation prior to taking on assigned position; turn in with 90-day review.
2. Oversee remedial training of clinical staff, as needed during ride-alongs and feedback from involved parties.
3. Maintain and use clinical skills and expertise in the care of patients.
4. Ensure the general and clinical orientation of all new team members to include
completion of clinical competencies in a timely manner.
5. Perform evaluations in a timely manner. Ensure all documentation required for employee files is forwarded to Human Resources in a complete and timely manner.
6. Ensure all staff attend mandatory in-services.
H. Maintain knowledge and skills in the area of employee management and discipline. Maintain positive working relationship with the Director, Human Resources to manage recruitment, hiring, discipline and management of team personnel, including work-related injury reporting.
I. Make periodic visits to patient/family units and care partners to assess whether appropriate and adequate psychological, emotional, and spiritual supports are provided to the patient/family unit, as well as for service recovery.
J. Maintain close, cooperative relations with other Hospice leadership including Clinical Services, Support Services, Finance, Volunteers, Education, Care Navigation Center, Business Development, etc.
K. In conjunction with the Business Development department, develop working relationship with care partners, including key staff in nursing facilities, hospitals, physician practices, ALFs and family care homes within team area to facilitate contract implementation and continuing quality improvement. Make quarterly rounding visits, as well as service recovery visits.
M. Attend all meetings and work groups as assigned or requested by Regional Directors.
N. Responsible for implementation and compliance of Hospice Policies and Procedures, awareness of Federal and State Hospice rules and regulations, and Joint Commission standards.
O. Follow up ongoing QA/UR monitoring through routine chart review, review of satisfaction surveys and ORYX reviews. Coaching and counseling of staff expected as indicated/necessary.
P. Conduct patient/family unit and care partner education sessions as needed. Promote Hospice services to community organizations. Seek opportunities for development and enhancement of Hospice relations with the community.
Q. Develop close, interactive relationships with Provider Relations and VCRMs, to improve access to care, resolution of patient care problems, and quality communication on hospice service.
R. Promote a positive image of Hospice in the community at all times.
S. Responsible for timely submission of staff and patient data, to include synchronization of tablets and remote documentation.
T. Responsible for oversight of and appropriate utilization and documentation of higher levels of care.
U. Perform other duties as may be requested by the Regional Director, Operations.
V. Required to comply with all Covenant Care Policies, Procedures and Personnel Policies, including those specifically addressing disaster preparedness and response.
W. Determines the patient is hospice appropriate, documents to the patient's terminality, and establishes and maintains the care plan for medically necessary assessments and services. This applies to determinations and consultations with members of the IDG, including the attending physician or hospice physician, in all circumstances in which the level of care is to be assigned as short-term inpatient care or continuous care, and includes all actions required in Covenant Care’s policies and procedures regarding the placement or removal of a patient from those categories of care.
X. As a member of the Interdisciplinary Group, is responsible for determining and documenting the level of care status of patients, and in particular, inpatient level of care, to ensure the patient is appropriate and meets the criteria for short-term care needed to manage pain and symptoms in an inpatient facility. This responsibility includes appropriate management and documentation with the patient’s change in status either to short-term inpatient care or removed from that status, and all monitoring of the care during the inpatient period. This same requirement applies to management and documentation of patients who are placed on a continuous care level of care. Strict compliance with all rules and regulations, including Covenant Care’s policies and procedures, is required.
Special Demand/Physical Requirements:
Position may frequently require medium to heavy lifting. Normally, such lifting will include, but is not limited to, assisting with transfer/repositioning of the patient in the house or nursing facility. This position is also mentally demanding because of the continual need of assessing and evaluating certain aspects of patient’s care. In addition, this position requires considerable time traveling in a motor vehicle in connection with performing the essential functions of the position.
