We are seeking a Clinical Nurse Case Manager for the Baltimore City Health Department (BCHD) Sexual Health and Wellness clinics . Will work under the supervision of the HIV Clinical Nurse Manager and will perform duties related to medical case management as a member of the health care and support service team providing services to HIV infected individuals. The primary duties include case management for a select panel of BCHD HIV continuity patients.

Specific Duties & Responsibilities:
Case management a panel of HIV continuity patients including monitoring and educating client about medication side effects, disease process, and lab values

Conduct psychosocial assessments, identify needs for interventions and facilitating the patient's ability to access recommended interventions, and develop treatment plans.

Counsel and refer patients with complex health problems in consultation with providers

Coordinate and track medical referrals

Triage patients presenting with acute issues, by phone or in person

Collect specimens and coordinate with lab when necessary

Administer treatments and medications as appropriate

Assist with prescription refills

Review lab work and triage to providers as appropriate

Provides patient education on HIV, STD, hepatitis test results, disease transmission, and risk reduction

Obtain sexual and behavioral risk history

Provide HIV treatment adherence counseling including: adherence assessment/Plan; HIV/HAART Education, pill box; regular monitoring of adherence and tolerance to medications; pharmacy monitoring of prescription completion by patient

Complete all charting in medical records per protocol

Provide general health education and support for patients

Assist with clinic initiatives as necessary

Help facilitate patient education and support groups as necessary

Assist with interpretation for Spanish speaking patients when medically necessary

Additional administrative tasks as necessary
Physical Requirements

Work produced is subject to precise measures of quantity and quality.
Near vision to see objects clearly within 20 inches.
Sharp focus to adjust vision when doing close work that changes in distance from eyes.
Full spectrum vision to identify and distinguish color.
Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s), for example, using a keyboard.
Handling by seizing, holding, grasping, turning or otherwise working with the hand or hands, but without finger dexterity.
Occasionally lifting, carrying objects weighing 20 lbs. or less.
Occasionally pushing, pulling objects weighing 20 lbs. or less.
Ability to move about without restriction.

Minimum Qualifications

Individual must be a registered nurse, licensed in the State of Maryland or state where practicing.

Current CPR certification required.
Must maintain current licensure and certification during duration of employment.

Acceptable completion of the JHH credentialing process is required prior to start date

Preferred Qualifications

Bachelor's degree in nursing or related discipline preferred.

Phlebotomy and injection skills
Knowledge of STD/HIV/TB/HCV disease, treatment, and resources
Ability to work with culturally diverse patient, staff and community populations
Ability to work in multidisciplinary, multi-organizational team setting
Strong organizational skills with attention to detail and follow-through
Self-starter with the ability to work with minimal supervision
Strong leadership potential
Spanish speaking skills
