The Portage County Health & Human Services department is hiring a full-time Community Health Planner!

What is the Summary of the Position?

Under the direction of the Portage County Health and Human Services-Division of Public Health Manager this position serves as a public health strategist within a nationally Public Health Accreditation Board (PHAB) accredited local health department. The health planner position works in collaboration with community groups, other agencies/departments, and resources to develop and implement community health services or policies by conducting assessments, capacity building, planning, implementation, and evaluation strategies. This position supports goals and objectives for identified health focus areas of the Community Health Improvement Plan for Portage County and Calls to Action for the Portage County LIFE Report. A good understanding of the essential services and core functions of public health is expected. The health planner position is expected to promote the health of our community and facilitate policy and planning, Expectations of the health planner position include the ability to:

  • Develop and implement interventions, policies, and systems to promote practices that support positive public health outcomes and resilient communities.
  • Use a variety of accessible, transparent, and inclusive methods of communication to convey and to receive information from the public and stakeholders.
  • Coordinate planning and serve as a source of information and expertise in the development and implementation of policies affecting public health.
  • Foster and support community involvement and partnerships in development, adoption, and implementation of policies affecting public health, including engagement of diverse populations and consideration of adversely impacted populations.
  • Develop a written community health assessment and improvement plan at least every 5 years, by assessing applicable data, developing measurable health objectives, and partnering with persons, agencies, and organizations to cultivate community ownership throughout the entire development and implementation of the plan.
  • Engage members of the community in assessment, implementation, monitoring, evaluation, and modification of community health planning.
  • Promote land use planning and sustainable development activities to create positive health outcomes.
What are the minimum qualifications?

  • Graduate from accredited college or university with a bachelor's degree in public health, public health education or community health, public health nursing, or related field. A master's degree in public health preferred.
  • At least one year of professional work experience in public health/community health field required. Health education in a community-based setting preferred.
  • Certified Health Education Specialist (CHES) required or must be obtained within one year of employment and maintained during employment. CHES certification not required for candidates with a master's degree in public health, public health education, community health, public health nursing, or related field or for a candidate who has at least 16 graduate semester credits towards a master's degree in public health, public administration, or health administration and completes the master's degree in within their first year of hire.
  • Proven ability to organize/facilitate groups.
  • Proven ability to plan and complete projects.
  • Experience in planning and project management preferred.
  • Experience in grant writing and grant monitoring preferred.
  • Experience in data collection/analysis preferred.
