Position Title: Community Liaison

The Community Liaison (Sales Representative) promotes Haven’s unique programs and services, which is positively influenced by the execution of sales processes including prospecting, networking, strategy development, and the formulation of varying techniques for attracting and retaining residents within the Community. The Community Liaison is responsible for growing the census and being a positive reflection of our company, while being heavily involved in the community. The Community Liaison develops and implements a regular, structured program of contact and outreach to identify new referral sources and to maintain ongoing relationships with partners, physicians and referral sources that will increase census and preference for our services.

Responsibilities:

  • Develop relationships with qualified referral sources, follow up with prospects with a great sense of urgency, conducting tours with potential residents and their families
  • Monitor census reports to determine needs of the Community
  • Determine primary decision makers and proactively build positive, solid working relationships with members of the medical community and partners
  • Promote and position our services through regular community outreach in a manner that illustrates and reflects our quality of care and unique capabilities
  • Work to build our brand as the preferred provider in our service area to achieve and exceed goals for growth and census management
  • Make regular, planned calls to secure new referrals, build census, and meet established goals on new and existing referring physicians; facilities; case managers, discharge planners and social workers in hospitals and skilled nursing facilities; and other health care providers
  • Conducts initial meeting with the patient and family giving an overview of our services and philosophy and providing tours of our communities
  • Develop and maintain a working relationship with multiple referral sources and provide effective education of services
  • Build brand awareness throughout referral base and monitor community, PCP offices, hospitals and other referral sources
  • Stay current with industry and marketplace changes and opportunities for competitive advantage
  • Collaborate and communicate with team and cross-functional partners to fully meet customer needs
  • Maintain knowledge of and commitment to Haven’s philosophy of care
  • Attend after hour/weekend functions such as vendor booths, fundraisers, mixers, etc. that are the responsibility of the marketing department
  • Ensure compliance with all state and federal legal and regulatory requirements
  • Additional responsibilities as assigned

Job Types: Full-time, Part-time

Pay: $41,600.00 - $100,000.00 per year

Schedule:

  • 8 hour shift
  • Monday to Friday
  • Weekend availability

Supplemental pay types:

  • Bonus pay
  • Commission pay

Work Location: On the road
