Job Title: Community Support Program Coordinator

Location: Mansfield, CT
Located at our new state-of-the-art Windham Regional Health and Wellness Center at 140 North Frontage Road, Mansfield Center, CT.

Job Description:
United Services is seeking a Master’s level Community Support Coordinator for the Community Support Division of a large community mental health center working with adults with serious mental health issues who may also have substance use disorders and/or co-morbid medical conditions. Provide leadership and supervision to group of Bachelor’s Level case managers who provide skills teaching and case management services in a community based program. Ensure day to day operation of services to meet client need in compliance with all accreditation and funder regulatory guidelines. Work collaboratively with agency and community services and problem solve at a systems level. Provide early outreach, assessment, and transitional planning to hospital patients and community referrals. Monitor program and individual productivity and provide supervision to meet program outcomes. Provide guidance and crisis support to staff with respect to client issues and programmatic needs.

Community Support Program Coordinator Job Requirements:
Master’s Degree in Counseling, Social Work or Human Services field required, plus four years direct client experience or 2 years supervisory/coordination experience required.

Salary Range:

• $48,776- 55,186
• $51,245- 57,979 Bilingual

Schedule: Monday - Friday 8:30 am - 5:00 pm w/ 1 shift 10:30 am - 7:00 pm
