Coordinate and participate in all aspects of resident care while promoting independence, to ensure quality of life. Supervise; provide leadership and direction to all Caregivers / Medication Technicians. Carry out supervisory responsibilities in accordance with the organization's policies and applicable laws.

Essential Functions and Work Duties:*
The following essential functions of this position are performed personally, in cooperation with the supervisor, and/or in coordination with other staff. Additional work functions and duties may be assigned.
*
Maintain compliance with NRS 449 for Residential Facilities for Groups.
*
Provide resident care in accordance with HAND Senior Living policies, procedures, and facility standards.
*
Maintain confidentiality and function as a role model to other staff.
*
Provide appropriate care for residents through assisting with all Activities of Daily Living (ADL) and medication/treatment regime while promoting independence.
*
Initiate contact with physician, emergency service and family in event of change in physical/mental status and provide essential care.
*
Transcribe, execute, and coordinate physician orders to ensure appropriate care.
*
Coordinate resident appointments to expedite care. Assist with developing, implementing and evaluating all areas of residents’ care and needs to ensure quality of care.
*
Assist with resident admissions, discharges, and transfers for smooth transition.
*
Develop, monitor, and maintain updated Care Plans.
*
Participate in quarterly interdisciplinary meetings to enhance residents’ quality of life.
*
Responsible for maintaining accurate and updated documentation and records to ensure good communication and thereby provide for effective continuity of care.
*
Instruct and supervise staff in HAND Senior Living policies and procedures to ensure proper technique.
*
Supervise, evaluate, coach, and assist staff with work performance to maintain quality care.
*
Prepare schedules and update assignments for staff to ensure adequate care.
*
Promote safety in care and surroundings to prevent injury to residents or staff.
*
Communicate with the Administrator daily to ensure continuity of communication.
*
Maintain knowledge of and implement emergency measures to ensure residents’ safety and comfort in the event of accident, fire, or disaster.
*
Participate in annual training as required by NRS regulations.
*
Identify and suggest innovative approaches and participate in implementing performance improvement opportunities.
*
Perform administrative duties such as filing, typing, copying, and other office duties as required.
*
Implement specific procedures and programs; insure confidentiality of all information.
*
Cooperate with, participate in, and support the adherence to all internal policies, procedures, and practices in support of risk management and overall safety and soundness.
*
Other Functions and Work Duties:*

Perform other duties as from time to time may be determined necessary for the well-being of the total operation.
*
Coordinate specific work tasks with other personnel within the department as well as with other departments in order to ensure the smooth and accurate flow of work and information.
*
Perform tasks which are supportive in nature to the essential functions of the job, but which may be altered or re-designed upon individual circumstances.

Qualifications and Skills Required:*
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience:*
- Must be eighteen (18) years or older and be registered as a Licensed Registered Nurse or Licensed Practical Nurse in the State of Nevada or able to obtain a valid license within sixty (60) days of employment. The license must be current and in good standing with the Nevada Board of Nursing.
*
Must possess a valid CPR and First Aid card.
*
Minimum of two (2) years of previous supervisory experience.
*
Prior experience in an assisted living community is preferred and/or relevant experience in skilled nursing.
*
Other Skills and Abilities:*
- Ability to read, write and speak in English.

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

Physical Requirements:*
- Stooping - frequently bending body downward and forward by bending spine at the waist, requiring full use of the lower extremities and back muscles.

Kneeling - frequently bending legs at knees to come to rest on knee or knees.

Reaching - frequently extending hand(s) or arm(s) in any direction.
*
Handling - frequently seizing, holding, grasping, turning, or otherwise working with hand or hands.
- Regularly required to use hand to finger, handle, or feel, reach with hands and arms, and talk or hear using telephone, computer keyboard, and mouse while seated at a desk.

Specific vision abilities required by this job include close vision, depth perception and ability to adjust focus.

Medium work: Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.

The worker is not substantially exposed to adverse environmental conditions.

The worker may potentially be exposed to infectious diseases.
