Diversion Specialist
Hours: Monday - Friday 8:30am - 4:30pm
Location: Passaic County (Local travel is required, as needed)
(This is a Grant based position, there is no guarantee the grant will be renewed (Grant runs from 7/1/22 to 6/30/23)
GENERAL SUMMARY:
The goal of the program is to divert families and individuals from the shelter system by providing case planning and creative housing solutions within 7 days of engagement. The program is a collaborative partner under the Passaic County Continuum of Care committed to addressing the needs of homeless or at-risk families in Passaic County. The case manager functions as a conduit to process assistance applications and assist financial assistance is rendered to avoid their entrance into the shelter system. The case manager is responsible for the coordination, development and planning goals to promote self-sufficiency to attain permanent housing.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Case managers may be assigned walk-in duties for CES programs, in addition to caseload.
- Meet face to face and/or virtually with clients. Determine client needs based on interview, develop a budget, develop Housing Stabilization Plan, provide housing stabilization services intended to meet basic essential needs.
- Work with clients to identify long-term solutions to recurring problems. Network with parishes, community/government organizations to effect maximum utilization of available resources to meet basic essential needs.
- Develop and maintain case management plan, case files and monthly statistical reports. Enter all client information into AWARDS.
- Provide follow-up services as needed.
- Represent Agency at Provider meetings and report back to Supervisor.
- Share housing opportunities with clients through flyers, group meetings, etc.
- Work independent with accuracy and complete adherence to policy and procedures.
- Responsible for other duties that may be assigned from time to time to support the effective and efficient operation of Catholic Charities.
QUALIFICATIONS:
- College degree preferred; 1 year of related work experience
- Complete work with accuracy and meet deadlines.
- Knowledge of community resources is highly desirable.
- Ability to utilize necessary technologies to provide and document services virtually.
- Communicate effectively, both verbally and in-writing.
- Basic computer and software skills and experience.
- Valid Driver's License
