We're looking for someone to join our team as an ECM Outreach Worker who:
The Enhanced Care Management Outreach Worker is a member of the care team. The Outreach Worker is responsible for promoting awareness of and access of programs within the community to assist families when facing social determinants of health. The Outreach Worker will work closely with Program Care Coordinators and RN-Case Managers in order to provide support to patient caseloads. The Outreach Worker will act as a liaison with community partners to further Clinica Sierra Vista & program goals for the community and patient education. The position has a strong emphasis on health promotion, disease prevention, and community capacity building and works within a harm reduction framework. These duties are accomplished as a part of an interdisciplinary team of professionals whose objective is to provide health care and follow-up for patients with chronic diseases. The Outreach Worker will supply patients' information-referral materials to community services.
2. Comply with established Clinica Sierra Vista policies.
3. Uphold the mission of Clinica Sierra Vista.
4. Identify programs and resources needed to support outreach activities by developing and maintaining informal and formal community networks.
5. Provide collaboration with Care Coordinators and RN-Case Manager on health education outreach and health promotion and preventive efforts. Responsible for documenting in patient's electronic health care record on services referred.
6. Determines client status by assessing the social economic position, physiological outlook, housing, financial and legal needs.
7. Prepares intake and assessment dashboards and maintain accurate and timely client documentation.
8. Fulfills all other documentation requirements as required by administrator of grants for supportive services.
9. Learns and maintains knowledge of the locations and movements of persons, problems and resources used, as well as other patterns or characteristics of the sub-culture.
10. Ensure all available and required resources are in place and are use appropriately.
11. Encourages community development by assisting individuals, groups and communities to articulate and advocate for health and related services.
12. Maintains personal contact with social service agencies and community services.
You'll be successful with the following qualifications:
- Education: high school diploma or GED equivalent.
- Experience working with homeless and persons from diverse cultures and income levels.
- Knowledge of community resources desirable.
- Knowledge of socio-economic environmental and cultural factors affecting patient's health care.
- Intermediate skill level in the use of a PC and related software applications (MS Office, Access and Excel).
- Excellent verbal, written communication and listening skills.
- Proficiency in basic mathematics and analytical comprehension.
- Ability to comprehend complex program requirements.
- Must adhere to Clinica Sierra Vista’s employee health/immunization requirements or provide a valid exemption request for subsequent approval.
