Employment Support Professional (ESP) - Saranac Lake Job at Citizen Advocates, Inc
SUMMARY/OBJECTIVE:
Employment Support Professional supports individuals at their job to learn work tasks, find solutions to barriers, teach soft skills related to work, etc. The job coach reports to the Supported Employment Program Manager.
Essential Functions (Job Duties)
- Coach, mentor and support individuals at their jobs. Learn the individuals job tasks and responsibilities so you can train them to complete tasks correctly, efficiently and timely.
- Assist individual in all aspects of their work including but not limited to: Learning and adapting to new work environment, fostering natural supports, proper communication with supervisor and co-workers, learning and understanding work rules and policies, etc.
- Model appropriate work behaviors.
- Identify barriers and find creative solutions to help the individual succeed at the job.
- Work with individuals at their job sites to help them overcome obstacles to maintain employment successfully.
- Talk to individual's employer on a regular basis to ensure no issues at work. If/when issues are communicated or identified, work with the person and supervisor to develop strategies for overcoming barriers.
- Maintain contact with person's plan writer (Employment Specialist or Job Developer) to keep them apprised of person's progress or any concerns that might affect the person's success at work.
- Perform formal and informal assessments to learn about the person, their learning style, challenges and strengths.
- Complete appropriate service documentation for every service provided in a timely manner.
- Review person's employment plan and work on goals identified in plan.
- Complete all forms required for billing purposes; and any other forms required by the agency or supervisor.
- Complete any documentation corrections as needed in a timely manner.
- Protect individual's confidentiality.
- Participate in at least 24 hours of Innovations trainings within the first year of employment, and at least 6 hours annually thereafter.
- Participate in all required and recommended agency trainings.
- Professionally represent Citizen Advocates (Career Visions) with families, employers, other organizations and the community as a whole.
- Adhere to all policies and regulations set forth by Citizen Advocates, Inc.
Required Education and Experience
- High school diploma or GED.
- Must have valid driver's license and reliable transportation.
