Responsibilities:

• Manage and track patient referrals
• Assist patients with scheduling appointments with specialist or for screening/diagnostic tests
• Connect and direct patients to appropriate referral resources
• Obtain referral authorization from insurance companies
• Ensure that the patients/families have adequate understanding of referral procedures and the reason for referrals or tests
• Provide education and support to patients/families about the importance of keeping the referral appointments
• Act as the referral system navigator and point of contact for patients/families
• Act as a liaison between the health center and external health care facilities, managed care companies or other organizations to facilitate patient referral and coordination of care
• Generate urgent referral status reports from EMR
• Generate weekly referral and test tracking EMR reports
• Track patient referrals and tests ordered and provide patient follow-up
• Assist in obtaining consultation/specialists reports and/or test results back to the Health Center
• Follow up with patients post-hospitalization/ER visit
• Collaborate with social worker to provide assistance and support to patient with financial or health insurance issues
• Connect patients/families to on-site or community resources
• Participate in quality improvement project and other related clinical QI activities
• Assist providers with translation/interpretation when needed
• Assist nursing staff with orientation and training of volunteers and trainees
• Assist with nursing area activities such as taking vital signs, height and weight and performing other basic clinical procedures as needed
• Perform other duties as assigned

Minimum Qualifications:

• Two years community college or equivalent
• Health care related degree preferred
• One to two years community health or health care experience
• Bilingual in English and Chinese (Cantonese and Mandarin)

Compensation and Benefits:

• Salary: $40,000.00 - 45,000.00 per year
