Overview: The Social Worker provides full range of hospice social work service and bereavement support to patient and families in the care center, home and institutional settings. They work with interdisciplinary team to provide directly, or through coordination of team members, appropriate assessment of and support to patients and families. Also, this position serves as liaison with other community resources; assists staff, patient’s physician and volunteers in understanding significant social and emotional factors affecting a patient and family during terminal illness. The Social Worker provides assessment for normal and complicated grief and provides bereavement counseling as appropriate. Due to business needs, Social Worker will be required to work different shifts, teams and location as needed. If you are compassionate and detail oriented, then we want you!

**This position will cover the Orange Park Care Center, Palatka Care Center and East Coast Hospitals.**

Location: Orange Park

The Orange Park Care Center is Haven’s fifth free-standing care center serving north Florida is a state-of-the-art facility that allows us to expand our services to better meet the end-of-life and palliative care needs of patients and families in the area. The care center is located at 745 Blanding Blvd, Orange Park.
