Health Information Management (HIM) Clerk - PRN Job at Denver Health
Releases and copies medical records for outside agencies, patients, courts etc. in an established process and procedure, following state and federal laws for legal release of health information. Under minimal supervision, audits the medical record to ensure completeness and legibility. Pull medical charts to prepare clinical lines. Files loose paperwork daily and accurately. Maintain the four-part chart accurately. Duties include release of information for medical record requests and answering customer questions via telephone. Provides administrative assistance to the Denver Sheriff Health Services Unit. Maintain a high level of Confidentiality. Maintain a professional relationship with Denver Sheriff Health Service’s staff, along with Denver Health.
- Receipt and processing of health information requests on a daily basis. Including preparing medical records for various departments based on next day schedule lines. Deliver medical records to health services staff as requested.
- Working with other facilities for exchange of health information.
- Processes request and or releases for health information from Internal Affairs, Adult Probation, Social Security Administration, State Hospital, Correctional care facilities, patients, medical facilities, government and entities.
- Receive and places telephone calls from/to other agencies, patients, lawyers, and other correctional care facilities.
- Creates new charts or order charts from other locations as needed Duties include: pulling patient’s charts for copying, ensuring charts are scanned in/out to correct location in tracking system following standard procedures.
- Performs spot audits, purges shelves when they exceed capacity. Prior to sending charts to warehouse ensure completeness.
- Reviews each document for accuracy. Detects and corrects chart misfiles.
- Navigate Epic, Jail Management System, and scheduling systems.
- Analysis of health records. Assembled health records are analyzed for deficiencies as defined by state and federal HIM record requirements, and hospital polices. Deficiencies are tagged for completion and entered into hospital software.
- Ensure proper protocols for patient transfers are followed for continuum of care. Coordinate with Health Services Staff and Denver Sheriffs Department to ensure proper documentation on each transfer.
- Forwards health records requests for patients no longer in custody to Denver Sheriffs Department for billing, release and collection.
- Other duties as assigned.
- Prefer individuals with HIM and computer experience
- Ability to interact well with customers.
- Self motivated, multitasker, detailed oriented
- Excellent written and verbal communication skills
- Knowledge of terminal-digit filling systems sufficient to maintain records.
- Demonstrates knowledge and applicability in complying with State and Federal regulations, HIPPA, Denver Sheriff Department, NCCHC, and ACA regarding release of information and privacy of patient records.
Computers and Technology
- Knowledge in the applications of the following: medical record imaging, Microsoft Outlook, and Microsoft Excel.
