Job Summary:
Vitability Health is seeking for an advanced practice provider who is willing to be responsible for providing House call to seniors and patients who require patient monitoring. Also, who has a passion for improving the way care is delivered and who wants to get back to the fundamentals of good medicine. Our advanced practice providers should be eager to take on professional development opportunities and deepen patient relationships.
Responsibilities:
- Travel to a patient’s home to preform house call visits and provide evidence-based medical care.
- Provide medical consultations, diagnoses, and treatment plans for patients through telehealth platforms.
- Conduct comprehensive patient assessments, evaluate patient histories, and order and interpret diagnostic tests.
- Educate patients and their families about their health conditions, treatment options, and self-management strategies.
- Utilize remote patient monitoring technology to monitor and manage patients' health conditions.
- Stay up-to-date with best practices in telehealth and remote patient monitoring and participate in ongoing education and training programs.
- Ability to treat and manage the health and well-being of patients (from adults to seniors) as it pertains to their wellness, urgent medical issues, and chronic illness in a value-based primary care relationship.
- Strong work ethic with effective communication skills and a collaborative spirit.
- Attention to detail, especially in medical documentation.
Required Skills/Abilities:
- Active and unrestricted license and DEA to practice as a Physician Assistant or Nurse Practitioner in the state of Massachusetts.
- Physician Assistant or Nurse Practitioner must live in Massachusetts.
- Must be able to easily utilize a smartphone, tablet, and EMR.
- Dedicated to providing excellent healthcare and a top-quality patient experience.
- Strong belief in the revolutionary nature of house calls and passionate about improving (or better yet disrupting) care delivery.
- Consistently exhibit the highest levels of professionalism, integrity, accountability, and compassion.
- Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking.
- Ability to lift and carry equipment up to 20- 30 pounds.
