The Part-time Licensed Practical Nurse (LPN) provides and directs provisions of nursing care to clients in their homes in collaboration with the primary health care provider, and in compliance with applicable laws, regulations, and agency policies. Also, Licensed Practical Nurse (LPN) coordinates the plan of care in conjunction with other health care professionals. The home health LPN helps to achieve excellent clinical outcomes of care through the delivery of nursing process.

Home Health is a specialty- it allows the clinician (or nurse) to practice 1:1 nursing care and be totally engaged in the moment with their patient. You are not pulled in multiple directions with competing priorities. You are able to focus your attention and care on one person for whatever time you have scheduled with them.

You have better control over your time- your schedule, and your clinical outcomes. Because of this, you have a better work life balance. It is not shift work. You don't work a scheduled shift. Your day is based on the times you set up with your patients.

License Requirements

• Must have current unencumbered LPN license for the state of Wisconsin
• Active unrestricted driver's license and insurance
• Current CPR certification required.
• Current Vaccines for Covid-19 or medical exemptions required

Qualifications:

• 1 year of clinical experience - Home Health/ Hospice (required)
• Provide care to patients as outlined by the Plan of Care.
