Visiting Nurse Association of Johnson County is seeking a caring and committed full time or part time Home Health Nurse to provide skilled care and case management services while engaging and creating personal connections with our clients in their home, so they can live independently and safe. Our professional Interdisciplinary Team provides a range of services to clients that need assistance so they can enjoy a full life and remain independent in their own home.

Our Nurses:

• Enjoy working with a wide range patient needs based on a variety of acute or chronic illness, recovery from injury or surgery, or are otherwise home bound

• Work closely with all members of the patient’s care team – Home Makers, Aides, Social Workers, Therapists, Physicians, to identify and coordinate care needs
• Serve as a crucial communication resource for physicians

• Log observations, treatments and other information in our EMR on an iPad

Requirements

Qualifications:

• A sincere desire to help others
• Current Iowa Registered Nurse License

• Iowa driver’s license • Ability to communicate in English verbally and in writing

• US work authorization We also require (and can provide):
• Two-step tuberculosis test (we can provide at the start of employment)
• Per CMS regulations, provide proof of COVID-19 vaccination(s) or medical/religious exemption
• BLS Certification
• Mandatory Reporter Training
• Available to work Monday-Friday 8AM-5PM and includes Call & Holiday rotations.
