This is a PT Monday-Friday Days+ Rotating Call

Position Summary:

The Nurse Coordinator coordinates the care of patients in the hospice program. You provide initial consultation/education to patients and families, assists with intake operations and ongoing patient care as needed. Serves as a resource to staff, patients/families, physicians, hospital staff and community partners. Assists with the quality assessment and performance improvement plan. The population served is neonate through geriatric.

• Plans and delivers care to terminally ill patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation
• Effectively interacts with patients, families, care providers and other interdisciplinary group members while maintaining standards of professional nursing and clinical competency
• Collaborates with physicians and other professionals to plan care
• Serve as a clinical resource to hospice staff

Successful candidates:

Graduate of an accredited school of Nursing and possess current Kansas RN License

Previous Hospice experience preferred

Familiarity with medical software and equipment

Current Driver's license

Ability to build rapport with patients and families/caregivers

Strong problem solving and critical thinking skills
