Arizona Care Hospice is always looking to add to our staff of highly qualified, caring clinicians. Currently, we are looking for a Hospice RN Case Manager for full time or per diem!!

We opened in Fountain Hills in August 2015, expanding into the Payson area in 2016, and then into the Prescott area in 2019. With a combined 40+ years of experience in healthcare across our staff however, Arizona Care Hospice is uniquely qualified to provide unparalleled care.
In addition to professional experience, many members of our staff have also been family members and caregivers of those faced with a life-limiting illness; they have had first-hand experience of the challenges that can be associated with discussing end-of-life care options with loved ones, and that often accompany making the decision to choose hospice over curative medical treatments as a care option.

Responsibilities:
  • Administer nursing care to terminally ill patients
  • Diagnose and establish patient treatment plans
  • Monitor and report changes in patient symptoms or behavior
  • Communicate with collaborating physicians or specialists regarding patient care
  • Care for patients throughout end of life, active dying process and after death
  • Facilitate referrals to other healthcare professionals and medical facilities
  • Maintain accurate patient medical records
  • Provide advice and emotional support to patients and their family members
  • Driving required, patient territory includes entire Prescott and Prescott Valley
  • On call rotation required

Requirements:
  • One year hospice or home health experience preferred
  • Familiarity with medical software and equipment, we use Consolo
  • Ability to build rapport with patients
  • Strong problem solving and critical thinking skills
  • Ability to thrive in a fast-paced environment
