Join MILC’s team to make a difference by helping individuals with disabilities self-direct their long-term health care services as an Include Respect I Self-Direct (IRIS) Consultant (IC). As an IC you will help participants navigate the Medicaid Long-Term Care program by connecting them to community resources. Through those connections, together you will develop a plan for achieving participants goals related to their health, safety, housing, transportation, etc.
We are currently looking for a full-time, home-based IC to serve Fond du Lac, Winnebago, and Manitowoc counties. While this is a work-at-home position, it does require meeting participants in their homes and within the community. Duties may involve assessing participants residences on all levels, which may involve stairwells, to make observations of home environments. Ability to travel within the IRIS Consultant Agency (ICA) service area and statewide required. Possess a valid Wisconsin driver’s license, provide proof of auto liability insurance meeting agency minimum limits and be able to meet agency driving standards or an alternate effective means of transportation required.
Individuals with a social/human services or related background that reside in Fond du Lac or surrounding areas are encouraged to apply.
Occasional staff meetings will require in-person attendance in the Stevens Point office. Employees must be available via phone/computer for regular scheduled check-ins.
Under the general supervision of the ICA Program Director the IC is responsible for providing ongoing support and assistance to participants to ensure their health and safety.
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
DUTIES include but not limited to:
Provide orientation to new participants and inform them of their rights and responsibilities.
Complete necessary paperwork, answer questions, and educate participants on how to read and interpret their monthly budget reports.
Assist participants in identifying their long-term support needs, desired goals, and outcomes through informal or natural supports.
Accurately complete and submit to Department of Health Services (DHS) all required forms, assessments and documentation for review and approval as indicated by the IRIS program policies and procedures.
Assure program quality through various quality management activities that are carried out in partnership with the participant and DHS to include but not limited to personal experience outcomes, provider qualifications, participant emergency and back-up plan, critical incident reporting and service plan and budget monitoring.
Meet regularly to ensure participants outcomes are being met while staying within the IRIS budget and within the requirement of the IRIS program determined by DHS.
Maintain positive working relationship with ICA staff, outside agencies, and service providers to ensure effectiveness of services provided.
Attends all ICA and DHS trainings, conferences and meetings as requested and required.
QUALIFICATIONS: The requirements listed below are representative of the knowledge, skill, and/or ability required.
18 years or older required.
Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or a closely related field REQUIRED AND have one (1) year of supervised experience working with seniors and/or people living with disabilities.
REQUIREMENTS:
Ability to become certified as a Long-Term Care Functional Screener (LTCFS) through completion of the Department of Health Services (DHS) web-based clinical certification
Completion of all required IRIS orientation and training courses
Understanding of IRIS philosophy and other Long-Term Care programs and the principles of self-direction, responsible financial management, and person-centered planning processes
Understanding of ICA services including behavior management, risk and needs assessments, service plan development and identification and reporting of critical incidents of abuse, neglect, exploitation, and fraud
Working knowledge of Microsoft Office, Outlook email, Teams, online portals, and internet
Competence in applicable areas of the Wisconsin Self-Directed Information Technology System (WISITS) and DHS SharePoint sites
Knowledge of community resources
Comfortable working with a diverse population and varied personal situations
Good organizational and time management skills
Ability to maintain confidential information including but not limited to financial data and consumer information.
Ability to pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Check
