Overview
This is an outpatient clinical LPN position that is responsible for overseeing training of all clinical staff in the implementation of clinical evidence-based interventions with patients at the Positive Health Clinic. Coordinates flow of medical and supportive services with all Positive Health patients. Supports integration of interventions such as Partnership for Health, S-BIRT, and Motivational Interviewing into clinical sessions, delivers client assessments and education around treatment adherence and risk reduction counseling, monitors fidelity, tracks service delivery and manages intervention data, coordinates booster trainings, and reports data. Responsibilities

  • Ability to navigate electronic medical records and patient database to track patient data and facilitate reporting of patient outcomes related to preventative interventions
  • Ability to engage patient in meaningful dialogue. Ability to prioritize workload and manage caseload.
  • Ability to work independently and function as an active team member. Working proficiency with computers, Microsoft Office products, and electronic medical records.
  • Provide support to the clinic by assisting in daily management of patient flow throughout the continuum of care rendered in the clinic to ensure optimum care delivery.
  • Coordinate training of all clinical staff in effectively delivering Partnership for Health (PFH) intervention and support integration of intervention into clinical sessions
  • Support and serve as a champion of SBIRT across disciplines at Positive Health, including integration within the clinical flow, tracking and monitoring the implementation of SBIRT services in clinic.
  • Support HIV/STD/HCV screening integration, coordination of clinical flow and follow up treatment for clients who test HIV and STD positive or need vaccinations.
  • Effectively deliver evidence-based interventions with fidelity around topics of prevention and risk reduction education, treatment adherence, sexual health and substance use
  • Provide ongoing treatment adherence support for patients, emphasizing the importance of adherence to medical appointments and, as appropriate, adherence to antiretroviral therapy and other medications.
  • Provide a patient-centered, holistic approach to patients with identified issues with adherence in order to achieve and maintain compliance with medical care and treatment.
  • Function as a liaison with patients and medical providers and assists in facilitating access to medical and social support services by participating in multidisciplinary case conferencing
  • Facilitate timely follow-up appointments with the member's primary care provider (PCP) and obtains relevant inpatient documentation for PCP review.
  • Enter and track patient data for patients receiving grant-funded interventions
  • Support Clinic Operations activities related to care coordination, responding to patient inquiries and feedback, patient education, overseeing medical supplies stock and equipment, ensuring optimal clinic flow
  • Support Continuous Quality Improvement (CQI) activities in the clinic and medical center.
  • Provide Health Literacy Assessments and documents appropriately in medical chart.
  • Project a positive image in representing and demonstrates effective interpersonal and communication skills.
  • Must be oriented to consumer satisfaction.
  • Perform other appropriate duties and participate in other special projects as assigned.

Position Requirements

This position requires a minimum formal education of Bachelor's Degree and a minimum of 3 years job-related experience.
1 - 3 Years HIV prevention and care related experience.
Bilingual English/Spanish highly preferred
License – NYS LPN
