Licensed Practical Nurse - LPN Job at Community Options, Inc.
Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 11 states.
We are seeking a Full-time, Part-Time, PRN Licensed Practical Nurse (LPN) in Manahawkin, NJ to deliver care to individuals with disabilities in a community setting, utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. The LPN effectively interacts with the patient, significant others, and other health team members while maintaining standards of professional nursing.
Starting pay is $27.00/hour
Essential Functions
- Provides treatment and health care procedures ordered by a physician and required by standards of professional practice or law.
- Participates in quarterly and Annual Individual Service Plan (I.S.P.) meetings for assigned participants.
- Creates and distributes Individual Service Plan Medical/Health Reports detailing participants general health, health care goals, and physician recommendations for each participant.
- Provides documentation regarding individual’s health status for various support people including family, guardian, staff, employment and other professional staff as necessary.
- Monitors participant's use of medications through Medication Administration logs.
- Makes referrals for needed medical services and ensures that follow-up care is pursued.
- Provides pre-service training for all new support staff on consumer specific medical needs.
- Provides quarterly in-service training for support staff.
- Monitors weight gain or loss and other health conditions of participants.
- Accompanies individuals to and from community, social, and recreational activities.
- Assures that health services supplies are available, and places supply orders when necessary.
- Advises supervisor of all concerns and/or issues related to the provision of nursing services.
- Assists in building a support network between the individual, family members and the community.
- Follows strict OSHA procedures regarding bloodborne pathogens.
- Participates in Circle(s) of Support for personal futures planning.
Non-Essential Functions
- Attends meetings as needed, based on consumer's needs.
- Attends conferences, seminars and training as required by licensure or supervisor.
- Other duties as assigned by the Executive Director.
Knowledge and Skills
- Excellent writing skills, and the ability to present facts and recommendations effectively in oral and written form.
- Ability to calculate figures related to medication dosages.
- Ability to handle multiple projects and perform duties under pressure of deadlines and tight work schedules.
- Ability to respond to crisis situations, exercise professional judgement, and use common sense.
- Ability to effectively and professionally communicate with program participants, other agencies, staff and members of the community.
- Commitment to and knowledge of community-based support for persons with disabilities.
- Ability to establish a comfortable and supportive relationship with a person receiving support.
Education and Work Experience
- Bachelor’s degree or equivalent experience in nursing field.
- Minimum of one year of experience providing nursing services to individuals with disabilities.
Required Licenses/Certificates
- Valid State nursing license.
- Valid drivers license with good driving record.
Working Conditions
- "On Call" 24 hours a day for medical emergencies, in cooperation with other management staff.
- Occasional overtime required.
- Frequent lifting, stretching and other physical exertion during positioning of consumer’s or equipment.
- While performing the duties of the job, employee travels by automobile, utilizing own vehicle, and is exposed to changing weather conditions.
- May occasionally assist with wheelchair transfer of non-ambulatory participants.
- May perform tasks that involve exposure to various medical conditions, communicable diseases, blood, body fluids, or tissues.
- Will work in varied residential environments based on assigned caseloads.
- Flexible schedule as determined by supervisor
