Responsibilities:
- Providing limited term skilled nursing services to individuals returning home from higher level care facilities
- Assessing, generating, and evaluating, patient care plans and patient progress
- Liaising with other members of each patients multi disciplinary care team
- Recording patient progress in HCHB via tablet
- Attending regular staff meetings
Requirements:
- Current NJ LPN License
- Reliable transportation, Driver’s License, and Insurance.
- Strong interpersonal and customer service skills.
- Ability to demonstrate knowledge of electronic documentation (HCHB or others)
- Bilingual a plus
- Additional training certificates a plus.
- Preference for nurses with prior Home Health experience including: Psych, Vent, Peds, Admissions
