Who We Are:

Glendale Place Nursing and Rehab, a 122 bed Long Term Care and Rehab facility, has been faithfully serving the Cincinnati community for many years with a history of providing excellent nursing care and successful rehabilitation services. We are currently seeking a MDS Coordinator, RN to join our team.

Expectations: The primary purpose of the MDS RN Registered Nurse is to collect data and implement a decision-making process to ensure that appropriate care will be provided to a resident. An interdisciplinary team will conduct the assessment. The RN will oversee the process, contributing input as needed and will certify the completion of the assessment. The assessment process includes completing the MDS, RAPs and care plans.

Guidelines for RN MDS Coordinator:

• The MDS Registered Nurse will be conducted according to the Federal/State regulations.
• 6 months - 1 year experience as an MDS Nurse required
• The tracking of MDS Registered Nurse due dates will be the responsibility of each MDS coordinator.
• MDS Registered Nurse will schedule care conference near the MDS completion date.
• MDS assignment will be divided into two parts as decided by the Administrator.
• With each MDS the following items will also need completion: RAPS as appropriate. Review and update care plan. Review quality indicators and address in care plan. Restraint/Appliance assessment. Psychoactive medication assessment. Review of nurses’ notes.
• The MDS Registered Nurse needs to monitor resident RUG scores and perform assessments as needed to maximize scoring.
• The MDS Registered Nurse will complete a discharge summary on any resident within his/her assignment.
• The MDS Registered Nurse will participate in the performance improvement program as directed by the Administrator and/or Director of Nursing.

Pay: From $65,000.00 per year
