SUMMARY
Under the direction of the Office Manager, the Patient Care Coordinator will aide in the day-to-day management of both new and current patients by providing remote support to various other team members in the field.
Key Responsibilities:
- Facilitate onboarding of new patients within his/her assigned buildings.
- Initiate and maintain patient charts.
- Coordinate nurse practitioner visits and referrals to specialist.
- Assist with Transitions to home and Chronic Care Management as directed.
- Facilitate telehealth visits as appropriate.
- Maintain and open line of communication with staff within the his/her assigned buildings.
Minimum Requirements:
- Certified Medical Assistant
- Requires a high school diploma or equivalent
- Reliable transportation
- Familiar with standard office technology to include telephone, email, and web-based browsers
Professional Skills:
- Excellent verbal, written, and interpersonal skills
- Self-starter with the ability to work independently
- Experience with electronic health records
