Medical Coordinator

Responsibilities Medical Coordinator:

• The Medical Coordinator is responsible for ensuring the optimal facilitation of medical care for the transient and/or inadequately sheltered individuals that are discharged from medical care facilities, or other agencies contracted, to Horizon Recuperative Care.
• In addition to providing high-quality medical coordination, this position will assist with community relations, admissions, and operational standards.

Qualifications of Medical Coordinator:

• Drivers license and Vehicle
• High school diploma or equivalent.
• 2-5 years of relevant experience.
• Possess intermediate skills using MS Word, Excel, and Outlook and are able to create basic spreadsheets and professional correspondence.
• Detail-oriented with good organizational skills
• Ability to interact tactfully, effectively, and professionally with other employees, and the general public, as needed
• Must be able to read, write and speak the English language sufficiently and comprehensibly enough to perform the essential functions of the job.
• Ability to work independently or part of a group.
