Job Summary:

The Memory Support Leader serves in two essential roles within the Memory Support neighborhood at Summit Square – 1. Oversee the daily operations of the memory support neighborhood. 2. Serve as the Life Enrichment Specialist (activities).

The Memory Support Leader works cooperatively and respectfully with residents, the neighborhood team, support teams, families, visitors, and volunteers to create a positive and pleasant environment in the Memory Support neighborhood. The Leader takes part in neighborhood programs with and for the Memory Support residents.

Residents’ rights and the role in helping to protect and exercise those rights.

Needs and abilities of elders and the challenges of diminishing physical, mental, and emotional capacity.

Dementia and how to care for seniors with memory and cognitive challenges.

Skills

Ability to meet the needs of geriatric residents.

Ability to work effectively with team members.

Ability to read and write, follow instructions, and communicate effectively in English.

Ability to assign, supervise, and review the work of others.

Ability to lead dementia in-services/workshops for Memory Support team members, volunteers, and families.

Basic computer skills including word processing and spreadsheet applications.

Education, Certification and Licensure Requirements

Bachelor’s degree in related field preferred, or equivalent combination of education and experience.

Qualified Activities Professional required, or ability to obtain qualification within six months of hire.

Certified Dementia Practitioner preferred.

Current Adult First Aid/CPR/AED certificate of completion, or ability to obtain qualification within three months of hire.

ServeSafe certification preferred.

Experience

One year of experience in recreation for a senior population in a healthcare setting preferred.

Supervisory experience preferred.

Essential Job Duties:

Job Duties – Memory Support Neighborhood Operations (Essential)

  • Resident-Centered Services: Develops and interprets standards of resident-centered services to promote the residents’ well-being and satisfaction. Guides the team as it establishes and implements the residents’ plan of care with the resident and his/her family or responsible party.
  • People Management: With assistance from the Assisted Living/Memory Support Nurse Manager (for clinical team members), recruits, selects, orients, trains and motivates all Memory Support team members. Supervises all Memory Support team members – including: Medication Aides, Certified Nursing Assistants, Direct Care Aides, Housekeeper, and Dining Services Assistant. Handles all team member relations issues.
  • Team Leadership: Builds rapport among the neighborhood team. Encourages self-directed teams, team problem solving, and conflict resolution. Partners with other departments. Leads by example.
  • Operational Management: Keeps the overall function of the neighborhood running smoothly. Schedules team members to best support the needs of the residents. Ensures that all needs are covered; minimizes overtime and PRN expenses.
  • Environment: Maintains a safe, comfortable, and functional environment that feels like home for the residents.
  • Financial Accountability: Prepares and monitors the Memory Support neighborhood’s annual budget. Seeks ways to control costs while maintaining quality.
  • Continuing Education: Attends in-service education programs and team conferences as scheduled; completes in-service programs as assigned.
  • Coaching and Developing Others: Supports a learning environment; keeps abreast of latest trends and new ideas; shares resources, knowledge, and expertise. Ensures that team members complete required trainings.
  • Performance Evaluation: Provides specific, frequent feedback to improve performance on job skills; addresses individual performance problems timely. Completes performance evaluations of Memory Support team members annually. All clinical team member performance evaluations are completed in conjunction with the Assisted Living/Memory Support Nurse Manager.

Job Duties – Life Enrichment Specialist (Essential)

  • Recreation Leadership: Facilitates the neighborhood’s ability to plan and carry out personal interests and social activities for residents. Trains team members in recreation practices and techniques.
  • Recreation Programming: Plans and carries out a variety of opportunities for meaningful programs and daily activities based on resident preferences. Works with the Life Enrichment Department to maintain a network of contacts with contractors, volunteers, and community organizations. Prepares and distributes monthly calendars. Plans appropriate leisure activities for residents that prefer to stay in their apartment.
  • Care Planning: Assesses and evaluates residents’ recreational needs, interests, and preferences. Works with resident, team members, and family to develop a plan of care. Participates in care plan meetings. Writes care plan notes and completes progress notes.
  • Special Events: Facilitates the organizing and implementing of special events including holiday celebrations and other special events involving residents, families, guests, and the community.
  • Materials: Develops and maintains a library of suggestions and supplies that grow everyone’s abilities to meet the recreational needs of residents. Orders recreation supplies and small prizes for various activities.
  • Quality Improvement: Participate in QA/QI initiatives and QAPI meetings.
  • Compliance: Ensures recreation operations are following federal, state, and local regulations and professional standards of recreation while honoring person-centered and resident-directed services. Ensures that appropriate recordkeeping and documentation are maintained.

Additional Job Duties

  • Dining: Joins mealtime conversations and maintains a warm, pleasant atmosphere while residents are eating. Assists residents who are unable to eat without support (with proper training and certification).
  • Laundry/Housekeeping: May provide assistance to ensure residents’ personal laundry is maintained in quality condition; may assist residents to wash, iron, fold, and sort their personal laundry. May provide assistance to ensure residents’ living spaces are maintained in quality condition.
  • May assist other neighborhood team members (within licensure).
  • May respond to call light requests and assess resident requests for assistance.
  • Perform other duties as required to ensure the well-being, safety and satisfaction of residents.

Work Environment:

This job operates the Memory Support environment and office space with normal lighting levels, temperature ranges, air quality, and ventilation and noise levels. During special events temperatures may vary with the season and location.

Work Schedule:

Average 40 hours per week mostly Monday through Fridays but evenings, weekends, and holidays may be necessary at times.

Physical Demands:

Capable of sitting, standing, walking, climbing stairs, reaching, turning, bending, stooping, crouching and kneeling, reaching over the head, grasping with both hands, and fine manipulation. Physically be able to carry, transfer, push and pull, and reposition residents with assistance or using appropriate equipment. Physically be able to lift up to 50 pounds unassisted. Normal eyesight or use corrective lenses to achieve it. Ability to hear and distinguish between normal tones and be able to perceive the nature of sounds. Ability to drive a 14-passenger bus (non-CDL) for outings.

Work Hazards:

Minimal hazards commensurate with general office conditions. Hazards related to exposure and handling chemicals. Possible exposure to bloodborne pathogens, although, universal precautions should be utilized at all times. Hazards related to transport and transfer of residents.
