Bear Mountain Healthcare is actively seeking an MMQ Nurse (RN or LPN) for a 24hr position with our facility located in West Springfield, MA.

Our comprehensive continuum of healthcare services includes Post-Acute Care, Short-Term Rehabilitation, Long-Term and Respite Care. Our facility is staffed with healthcare professionals who attend to every patient’s needs.

Job Duties:

• MMQ Nurse provides oversight and review of all documentation related to the MMQ process. Completes and forwards all admission and conversion MMQs as well as Semi-annual submission batches to the Massachusetts Office of LTC.
• Responsible for delivering care to residents/patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interacts with residents/patients, family members, and other health team members while maintaining standards of professional nursing to provide the highest level of care and safety to our residents.

REQUIRED CERTIFICATES, LICENSES, ETC. :

Current, unencumbered State License

MINIMUM QUALIFICATIONS:

- Must be at least 18 years old.

- Ability to meet all health, compliance, and competency requirements.

- Ability to read, write, and speak English; and communicate effectively verbally and in writing.

- Ability to complete required orientation as directed by the facility.

- Has successfully completed state required background check and is eligible to work in the facility.

- Computer literacy skills (Microsoft Outlook, Microsoft Office, Internet)

- High School Diploma

- Current, unencumbered MA nursing license.

- CPR certification.

-Must be fully vaccinated against Covid-19, per MA regulations.
