Patient Case Manager Responsibilities:
a. Provide inbound and outbound patient case management for patients and key operational contacts.
b. This Case Manager position is responsible for the clinical support initiatives to support caregivers and healthcare partners with remote patient care software and processes. The ideal candidate is a proven Case Manager with a demonstrated track record in managing patient health conditions and implanting processes to improve patient health. In this role, the Case Manager will report to the VP and perform the following essential responsibilities:
· Work as part of a team with care coordinators, case managers and other professionals to offer advice on cases when needed;
· Conduct initial patient care gap analysis and complete patient care-plan assessments;
· Create (re)assessment of physical, mental, cognitive, psychosocial, functional and environmental patient conditions as necessary to ensure proper care and treatment;
· Work with third-party care providers to review their patient care plans for all health issues;
· Coordinate with other care providers such as primary care and specialty physicians, to avoid unnecessary costs and miscommunications;
· Attend and participate in scheduled patient and client meetings; Support and educate clients, their care givers and related parties on issues related to the client;
· Maintain and update client files and complete required documentation;
· Administrative Responsibilities:
· Work cross-departmentally advocating for clients’ needs and requirements, ensuring the company prioritizes and designs the processes necessary to maximize clinical outcomes and grow client relationships;
· Responsible for projects, tasks and other duties as assigned;
General Responsibilities:
· Support and endorse our philosophy, programs and culture;
· Ability to work a flexible schedule;
· Build and maintain strategic and trust-based client relationships
· Liaise with patients’ families
Required Skills and Qualifications:
· Minimum 5+ years of healthcare experience;
· Completed LPN License Program
· Excellent communicator that can inspire with great observational skills and empathy for patients;
· Can exercise a high degree of initiative, sound judgement, discretion, and decision-making skills to address program requirements;
c. Perform actions required to satisfy caller in accord with our documented and shared instruction(s) and CSAT measures.
d. Document information in appropriate platform(s), as necessary.
e. Immediately notify leadership regarding any issues or barriers in completing caller(s) request.
f. Perform similar services in additional contact channels (email, chat, social media, etc.) or back-office functions as may be specific in the Work Order(s)
g. Provide a primary contact (account manager) and comply with requests for information in accordance with this agreement and Work Order(s).
