Responsibilities:
Consults with facilities on the monitoring, recording and reporting of symptoms or changes in patient's conditions. Supports facilities management by advising and assisting with policies and procedures and assessment support in accordance with all nursing standards of practice, state and federal regulations and guidelines.

Duties and Responsibilities

1. Consults with facilities on how to administer medications to patients and monitor patients for reactions or side effects.
2. Evaluates the delivery of services facilities. Assists management staff in the development of goals and
objectives and solutions to identified problem areas, which result in immediate and satisfactory response to concerns.
3. Provides assistance with and consults with facilities and nursing staff on patients' records, medical information and vital signs.
4. Acts as a "facilitator" to the Executive Director. Facilitation shall be in the form of advice, sharing ideas, listening to problems, identifying needs in terms of quality assurance, staff development and role modeling.
5. Consults with facilities on the monitoring, recording, and reporting of symptoms or changes in patients' conditions.
6. Consults and coordinates with facility team members to assess, plan, implement, or evaluate patient care plans.
7. Modifies and consults with facility nursing staff on patient treatment plans as indicated by patients' responses and conditions.
8. Monitors all aspects of patient care, including diet, medication, care and treatment plans and physical activity.
9. Instructs individuals, families, or other groups on topics such as health education, disease prevention, or the development of health improvement programs.
10. Maintains accurate, detailed reports and records.
11. Minimal travel including overnight stay as necessary.
12. Meets Expectations of Excellence for position.
13. Supports facilities, departmental goals and the overall objectives of the Company.
14. Complies with all laws, regulations and standards of ethical conduct.
15. Performs all job duties with a friendly positive and team-oriented approach.
16. Complies with all Company policies, procedures, rules and standards.
Qualifications:
  • Degree in Nursing, ADN, Diploma, B.S. or M.S.N. degree from an NLN accredited college or university.

Licenses/Certifications
  • Current unencumbered registered nursing license in the state of practice.
  • Valid CPR certification.
  • Certification as a Nurse Assessment Coordinator preferred.

Experience
  • Five (5) to seven (7) years' experience as a Director of Health Services (Director of Nursing) or Nurse Consultant. Healthcare, senior living industry or long-term care environment preferred.
  • Five (5) to seven (7) years' experience in clinical nursing.
  • Exemplary computer skills that include knowledge of the Microsoft Office Suite of products.
