MGH Primary Care APP Plus

Description

MGH Primary Care seeks to hire a full time Nurse Practitioner. The Nurse Practitioner will be responsible for managing a panel of patients as well as co-managing patients with select physicians in the practice.


PRINCIPAL DUTIES AND RESPONSIBILITIES:

Clinical:

  • The Nurse Practitioner (NP) will deliver preventative, follow up, and urgent care to patients of the Practice. The NP will Collaborate with MD partners, primary nurses, medical assistants, and support staff to deliver patient centered team-based care.
  • The NP will independently manage a panel of patients of his/her own, and be responsible for meeting quality, and panel targets as outlined by the department.
  • The NP may also co-manage patients within the practice, collaborative with designated physician(s).
  • As part of his/her role, the NP will perform and record the patient history and physical exam; form an assessment and plan, which may include ordering appropriate testing and imaging, and the interpretation of results and communication of the results to patients. The NP can also expect to consult with and seek physician collaboration, including specialists, when appropriate.
  • The NP is expected to complete documentation, act upon labs, imaging, and other studies promptly and in accordance with institutional and departmental guidelines, and policy.
  • The NP is also expected to respond to patient questions and messages in accordance with institutional and departmental guidelines, and policy.
  • Perform medically delegated aspects of patient care as agreed upon by nursing and medical administrators according to practice guidelines.
  • Maintain and practice in accordance with practitioner guidelines per state and hospital regulations.
  • Maintain and update clinical knowledge and skills based on current health care practice/research.
  • Maintain the standards and educational requirements specific to the position and licensure.


Administrative:

  • Complete patient notes and associated documentation within requisite time frame in accordance with institutional and departmental guidelines, and policy.
  • Evaluate the outcomes of patient care, using patient satisfaction information, MWI, ED & readmission rates and other data as made available by Primary Care Administration, MGPO and other appropriate sources within the institution.
  • Participate in and reinforce all practice-based process improvement efforts.
  • Serve as a resource to nurses, medical assistants, and staff for clinical questions/problems and participate in care when indicated
  • Participate in precepting student Nurse Practitioners in accordance with institutional and departmental guidelines, and policy.
  • Promote cooperation, support collegial relationships among all staff.
  • Promote an environment that is sensitive to the culturally diverse needs of patients and employees.

SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:

  • Extensive clinical in primary care, urgent care, and chronic disease management.
  • Experience managing patients independently, and co-managing patients with physicians.
  • Outstanding time management, organizational skills
  • Demonstrated ability to effectively interact with physicians and coworkers.
  • Strong written and verbal communication skills
  • Experience with common PC computer software including Microsoft Word, PowerPoint, Excel, Access and Outlook; experience using clinical data capture systems such as EPIC.

This position requires 5-7 years clinical experience as a Nurse Practitioner, preferably with expertise in primary care. Master’s degree in nursing required. Current Board certification as an Adult or Family Nurse Practitioner required.
