A Primary Care practice in Lake Zurich is looking for a Licensed Nurse Practitioner or Physician Assistant in Family Medicine and Internal Medicine.

Job Summary:

• Assists with all aspects of patient care, including diagnosis, treatments, and consultations in hospital and outpatient clinical office.
• Mentor for clinical personnel.
• Liaison with patients, staff, providers and management.
• Can perform as part of a treatment team or independently.

Duties would include but not limited to:

• Provide general and preventative care, conduct check-ups, treat illnesses, order lab tests, and prescribe medications for children and adults.
• Provide hospital coverage and rounding if you plan to take a full-time position.
• Educate patients about preventative care and the prescribed treatments
• Manage patients who do not require complex medical management or specialized treatment.
• Assess, plan, implement, and evaluate individual patient care appropriate to the age of patients
• Communicate professionally with patients served regardless of their age
• Complete charting and required paperwork in a timely manner and meet the quality measures
• Assist physicians in reviewing lab results of patients
• Active participation in the development/review of office policies and procedures.
• Promotes a safety-conscious work force and maintains good housekeeping practices.
• Flexible in schedule to accommodate changing needs of patients and business.

EDUCATION/EXPERIENCE REQUIREMENTS:

• Licensed as a Nurse Practitioner or Physician Assistant with a master’s degree from an accredited advanced nurse practitioner program.
• Two years of experience in family medicine or internal medicine are required. TIME SPENT ON TRAINING for degree or license requirements IS EXCLUDED.
• Proven excellent Quality and clinical Standards.

Hours:

A part-time position in outpatient office or a full-time hybrid position of both outpatient and hospital responsibilities.
