SUMMARYThe occupational therapist seeks to maximize a patient's functional independence and safety in the home environment. The occupational therapist is required to complete a thorough assessment for each patient relating to their functional status, activities of daily living, coordination, home assessment and other needs as determined by their medical condition. An individualized plan of care is developed and revised for each patient as appropriate to their rehabilitative needs. Teaching compensatory strategies, caregiver training, implementing adaptive equipment and utilizing sensory integration techniques are a few of the methods regularly utilized by the occupational therapist to support patients in meeting their goals. The occupational therapist actively collaborates with the interdisciplinary team to improve the lives of our patients, families and their communities.
ENTRY REQUIREMENTS
Bachelors degree from an Occupational Therapy program accredited by the American Occupational Therapy Association and the American Medical Association.
Current registration by the American Occupational Therapy Certification Board or eligibility for immediate registration, as well as current registration with the State of Michigan.
One year of clinical experience required, home health care experience preferred.
Physically move, position, and transfer clients up to 200# utilizing good body mechanics, lifting techniques and/or transfer devices.
Ability to communicate effectively in verbal and written English and by telephone.
Possess current Michigan motor vehicle license, ability to drive a car, and insured vehicle capable of transport to patient homes or other offices under various weather conditions.
Possesses a working landline phone in the home.
Meets MHH established requirements for proof of health status.
ORGANIZATION
Reports to appropriate Regional Manager.
Works with nursing, home health aides, other specialty services staff and office support staff within the department and consults/confers with other personnel in other departments such as Physical Therapy, Occupational Therapy, Nursing, Speech, and Social Services to meet health needs of the patient.
Is responsible to the individual patient's referring physician and works under the physician's direction and prescription.
Frequent interaction is required with other professionals, patients, patient's families and medical staff.
May work as a member of the Hospice team to help meet the needs of the terminally ill patient and his/her family.
SPECIFIC DUTIES

Uses effective customer service/interpersonal skills at all times.
Comply with standard precautions, infection control, and safety procedures.
Maintains CPR certification.
Provides initial and ongoing assessments to determine level of functioning including OASIS assessments at appropriate time points.
Provides initial and interim assessment and evaluation of the client’s level of functioning.
Problem identification and goal setting.
Develops the plan of treatment in consultation with the physician.
Provides occupational therapy in accordance with the plan of treatment.
Revises the plan of treatment as needed.
Coordinates with other disciplines and adheres to the plan of care
Provides patient/family education as appropriate.
Requisitions and manages equipment and supplies required to increase the client’s function and independence as appropriate to care.
Records in a timely manner evaluation data, treatments, clients’ response to therapy interventions.
Supervises occupational therapy assistant (OTA) and paraprofessionals as required by policy and state regulations
Participates in quality and performance improvement measures.
Responsible for such other job duties as may be assigned by the manager.
Hospice
Sets goals related to the needs of the hospice patient.
Consults with IDT members.
Provides equipment required to increase the patient’s function and independence.
Provides pain management, planning and evaluation.
Provides specialized hospice training.
