Outreach Support Specialist - Remote in Houston, TX. Job at UnitedHealth Group

Landmark Health , part of the Optum and UnitedHealth Group family of businesses, was created to transform how healthcare is delivered to the most medically vulnerable members in our community. Our medical group provides home-based medical care to chronically ill patients, many of whom are ill-equipped to navigate our overwhelming healthcare system.
The Outreach Support Specialist will be responsible for creating daily reports to support the Outreach team in follow up and outreach of non-engaged patients. S/he will also create call lists to support the Outreach Supervisor and serve as backup to the Office Operations Manager in creating full census reports as needed.
In addition, outbound and inbound calling to schedule patients for in-home clinical visits with a Landmark Health provider. S/he will be required to meet developing quality and production metrics to provide the highest level of customer service on each interaction. S/he will be responsible for learning the outreach model and adapting it to constantly changing patient interactions to meet business and patient needs. The ideal candidate must be goal oriented and willing to adjust to a fast paced and evolving environment.
This position is full-time, Monday - Friday. Employees are required to work our normal business hours of 8:00am – 5:00pm. It may be necessary, given the business need, to work occasional overtime or weekends. Our office is located at 14141 SW Freeway Sugar Land, TX.

Primary Responsibilities:
- Create Daily Census Reports on Non-engaged patients using health information exchange
- Develop weekly call lists for Outreach team
- Effectively communicate the advantages and value of the Landmark program to the target population
- Develop a thorough understanding of the Landmark clinical program
- Engage patients in the Landmark Health Program cohort
- Establish rapport with qualifying patients
- Maintain and ensure the confidentiality of all Protected Health Information
- Adhere to HIPAA guidelines and regulations
- Proper call routing to resolve patient issues
- Consistently meet production and quality metrics
- Flexibility to work extended hours to accomplish business goals
- Adhere to attendance requirements
- This position reports to the Office Operations Manager with a dotted line to the Supervisor of Outreach and Engagement.

Required Qualifications:
- High School Diploma / GED (or higher)
- 1+ years of customer service experience analyzing and solving customer problems
- Proficiency with Microsoft Word (create, copy, edit, send and save documents), and Microsoft Excel (create, copy, edit, send and save spreadsheets)
Preferred Qualifications:
- Experience working within the health care Industry and with health care insurance
- A clerical or administrative support background
- Experience working with Medicare and/or Medicaid Services
- Call center experience
Telecommuting Requirements:
- Reside in Houston, TX
- Required to have a dedicated work area established that is separated from other living areas and provides information privacy
- Ability to keep all company sensitive documents secure (if applicable)
- Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
