Job Summary:
Under the direction of the Director of Hospice and Palliative Care, the Palliative Care Coordinator will oversee the Palliative Care program, monitoring both programs for quality, opportunities for improvement, and growth. Collaboration with clinical staff and Providers to ensure the programs are meeting the needs of the community will be ongoing. The Palliative Care Coordinator will spend the majority of their time in the office setting but will also assess patients in the field. A high value will be placed on integrity and compassion and instilling this in all staff working in Palliative Care. The Palliative Care Coordinator will also be responsible for working with the team to ensure the Palliative program compliance with Federal, State regulations and NCHHHA guidelines.
Essential Qualifications:
An associate in nursing is required, bachelor's degree is preferred. A current and active NH RN license is required and at least 3 years of palliative, hospice, and/or home care is also preferred. A combination of education and experience that demonstrates the person has acquired the necessary competencies required for this position. Access to an operable, insured motor vehicle and a valid driver's license with an acceptable record of safe driving practice is required. Must obtain CHPN certification within two years of hire.:
