TITLE: Patient Care Coordinator

PROGRAM: South shore including Brockton, Plymouth and New Bedford Sexual & Reproductive Health Services

HOURS: Fulltime, Monday through Friday including some evenings

SALARY: $28.10/HR

BENEFITS: Medical, dental, holiday, paid earned leave, 403b, etc.

DUTIES: Patient Care Coordinator will work within a team and in partnership with patients to help address barriers to care and achieve better health outcomes by:

  • Providing patient-centered sexual and reproductive health education and counseling on topics including pregnancy options, abortion services, contraception, sexually transmitted infections, HIV, Hepatitis, PrEP, nPEP, gender affirming hormone therapy, overdose education and narcan distribution
  • Assisting with abortion navigation services including coordinating visits and ensuring continuity of care and linkages to community support services and resources
  • Developing and maintaining an up to date referral list of surgical abortion, medical, and social service providers
  • Assisting in outreach and engagement activities with internal and external programs including building relationships with other Health Imperatives programs, abortion navigation programs, and other community partners across our region
  • Completing Certified Application Counselor Certification and enrolling patients in health insurance
  • Providing short term health navigation and client linkages services to substance misuse treatment, mental health counseling, primary care, housing, crisis intervention, and other social and medical services as appropriate
  • Completing accurate and timely documentation in the EMR and report system
  • Acting as a liaison within HI programs and with community providers
  • Other duties as assigned

REQUIRED QUALIFICATIONS: Associates degree and at least 3 years of experience providing patient health education, care coordination, and/or or case management; strong client-centered skills; demonstrated ability to communicate unbiased reproductive health knowledge to adolescent and adult patients in an effective and culturally competent, non-judgmental manner; experience working with diverse populations; excellent written and oral communication skills in English; work independently as well as collaboratively within a team environment; computer literacy essential.

PREFERRED QUALIFICATIONS: Bachelor’s degree in health or human services or minimum of five years experience; case management or health navigation experience particularly regarding sexual and reproductive health and abortion related support services; demonstrated experience providing sexual and reproductive health education.
